<p>The last video dealt with case damning the existing treatment or recommendations to follow a high carb diet managed with ever increasing doses of insulin.  This video deals with the evidence for rather managing it with a low carb lifestyle.  There are two great studies for advocating this method.  </p>
<p>1:)  Dietary carbohydrate restriction in type 2 diabetes mellitus and metabolic syndrome: time for a critical appraisal<br />

Numerous authors including Richard Bertstein, Mary Vernon, Jeff Volek, Eric Westman</p>

<p>Conducted Multi-center trial in which 63 obese men and women were randomly assigned to either diet, 21 Low Carb, 21 Control, 21 High Carb for a one year study.<br />
This discussion focuses on type 2 diabetes but many of the principles will apply to metabolic syndrome and possibly to type 1 as well[6,7].<br />
1. Carbohydrate restriction improves glycemic control, the primary target of nutritional therapy and reduces insulin fluctuations.<br />
2. Carbohydrate-restricted diets are at least as effective for weight loss as low-fat diets.<br />
3. Substitution of fat for carbohydrate is generally beneficial for markers for and incidence of CVD.<br />
4. Carbohydrate restriction improves the features of metabolic syndrome.<br />
An important idea guiding current medical thinking is that clustering of seemingly disparate physiologic states, obesity, atherogenic dyslipidemia, hyperglycemia and hypertension, termed metabolic syndrome (MetS) suggests a common underlying cause. A recent review showed that carbohydrate restriction improves all of these markers[24].<br />
5. Beneficial effects of carbohydrate restriction do not require weight loss. It&#8217;s not simply a weight loss argument or due to people losing weight do they achieve the benefits.</p>

<p>Conclusion:<br />
There is now little evidence for the claim that a fat-reduced diet for weight reduction has any particular value beyond caloric counting [10].  Current dietary recommendations seem to be a major part of their problem rather than being part of the solution. Carbohydrate restriction, however, reverses or neutralises all aspects of the metabolic syndrome [20,21].</p>

<p>One of the questions I get quite often is can I drink during Induction or at all on Atkins.  This question is probably least hard for me to answer as I have given much of alcohol except for maybe a couple times a year.  With that, during Induction you definitely should not drink alcohol.  It is only 14 days long, and certainly you can last 14 days without drinking.  Why?  The same reason we restrict everything else in Induction &#8212; get you back to a simple diet or platform to readd foods, drop in ketosis, and start a new way to eat.</p>
<p>Some people can drink and lose weight still, but for others, it is a source of much trouble.  Two reasons this can happen:<br />
* all alcohol must be consumed for fuel first before burning anything else which can last up to a hour and half per drink.  So no fat burning while you are drinking alcohol.<br />
* Alcohol can lead increased hunger and decreased resistance to cravings.</p>
<p>Wait &#8217;til OWL phase, and figure out if you can add it back without gaining weight.  If you can lose and drink, make sure your selection are low carb options.  Figure out up front what and how many so you&#8217;ll know how long you will be off fat burning.</p>

<p>I responded that I averaged about 4.3 pounds per week during much of the weight loss phase, and over 3 pounds throughout the 15 months of the diet.  I advised against intentionally slowing down the diet by overeating or eating when you aren&#8217;t hungry.  I do HOWEVER recommend eating above one&#8217;s Basal Metabolic Rate to avoid starvation mode and crash dieting, such as water restriction.  I have read a story where the winner of the first Biggest Loser gained 35 pounds of water within 3 days after their weigh-in due to lost water weight.  Eat according to the plan and ~3-5x per day, and you should be fine.  </p>
<p>The whole point is to lose fat cells not muscle mass as much as possible.  If you are worried about lost muscle mass, keep exercising and gauge your long term performance as a key indicator.</p>

In this one, I deal with question of how long to stay on Induction and what the warning signs to progress to the Ongoing Weight Loss Phase. My response normally goes &#8211; you can stay on Induction for a really long time, but you should progress to OWL when you hit one of the warning conditions.</p>
<p>Here are the warning signs:<br />
1. poor exercise performance, prolonged fatigue, or tiredness<br />
2. Blood work comes back out of whack &#8211; One of mine came back borderline for globulin, the doc recommended I start eating nuts.  I had it rechecked shortly thereafter, and it was fine.<br />
3. Boredom with the food choices.</p>

Fauxtatoes are a great dish to have at Thanksgiving or for those who typically have to fight to get their veggies.  A couple of my kids and my wife even make cameo appearances in this video.  It&#8217;s the least I can do for all the taste testing I make them endure as I try new recipes.  My non-low-carbing family members even like this recipe as an alternative to starchy mashed potatoes.  </p>
<p>Try this recipe; I think it&#8217;ll be a hit with you as well!</p>
<p>Recipe:<br />
1 Head Cauliflower<br />
2.5 TBS Butter<br />
1/3 cup Heavy Cream<br />
1 Chicken Bouillon cube<br />
1 tsp salt<br />
White pepper to taste</p>
<p>Directions:<br />
* Cook Cauliflower until fork tender.  Drain well.</p>
<p>* Place cooked cauliflower and other ingredients (crush bouillon cube first) into the food processor.  Run on high until consistency of mashed potatoes.  You may need to scrape the sides of the bowl a couple times to prevent lumps.</p>

Like any diet, water is important to make you fill full, keep you hydrated for exercise, and to quench your thirst.  Its importance can not be stressed enough in a low carb diet though as its additional benefits, such as:<br />
* its role in metabolizing fat<br />
* its role in reducing Induction Flu symptoms<br />
* its role in flushing the toxins and by-products (like ketones) from the system through your kidneys</p>
<p>I deal with the quesiton of how much is required per day and even make some suggestions on how to make it more palatable for those hydro-phobic people like I was.  My suggestion is to look at water intake like a necessity similar to exercise.  For somethings you just need to get it over with and get on with life.  Eventually, it won&#8217;t seem as bad as it might when you are first starting, and given the prospect of lower than optimal weight loss, why wouldn&#8217;t you drink it? </p>

<p>Drinking Fewer Sugar-Sweetened Beverages May Lower Blood Pressure</p>
<p>ScienceDaily (May 25, 2010) — Drinking fewer sugar-sweetened beverages &#8212; a leading source of added sugar in the U.S. diet &#8212; may lower blood pressure, according to research published in Circulation: Journal of the American Heart Association. Increased consumption of sugar-sweetened beverages (SSB) has been associated with an elevated risk of obesity, metabolic syndrome, and type 2 diabetes, according to previous research. However, the effect of sugar-sweetened beverages on blood pressure is uncertain, said lead author Liwei Chen, M.D., Ph.D., assistant professor at Louisiana State University Health Science Center School of Public Health in New Orleans, La.<br />

<p>&#8220;Our findings suggest that reducing sugar-sweetened beverages and sugar consumption may be an important dietary strategy to lower blood pressure and further reduce other blood pressure-related diseases,&#8221; Chen said. &#8220;It has been estimated that a 3-millimeters of mercury (mm Hg) reduction in systolic blood pressure should reduce stroke mortality by 8 percent and coronary heart disease mortality by 5 percent. Such reductions in systolic blood pressure would be anticipated by reducing sugar-sweetened beverages consumption by an average of 2 servings per day.&#8221;</p>
<p>Researchers used data on 810 adults, ages 25 to 79, with prehypertension (between 120/80 and 139/89 mm Hg) and stage I hypertension (between 140/90 and 159/99 mm Hg ) who participated in the PREMIER study, an 18-month behavioral intervention study with a focus on weight loss, exercise, and a healthy diet as a means to prevent and control high blood pressure. At the start of the study, the participants drank an average 10.5 fluid ounces of SSB/day, equivalent to just under one serving. At the study&#8217;s conclusion, average consumption had fallen by half a serving/day and both systolic blood pressure (the pressure when the heart beats), and diastolic blood pressure, (the pressure between beats), had declined significantly.</p>
<p>After controlling for known risk factors of blood pressure, the analysis found that a reduction of one serving/day of SSB was associated with a 1.8 millimeters of mercury (mm Hg) drop in systolic pressure and a 1.1 mm Hg decline in diastolic pressure over 18 months. Researchers noted that this association was partially because of weight loss, but even after controlling for weight loss, the change in blood pressure was statistically significant.</p>
<p>Chen noted that American adults consume an average of 2.3 servings (28 ounces) of sugar-sweetened beverages per day. In this study, sugar-sweetened beverages were defined as drinks sweetened with sugar or high-fructose corn syrup including regular soft drinks, fruit drinks, lemonade and fruit punch.</p>
<p>Diet drinks were excluded. The study potentially has important public health implications, because even small reductions in blood pressure are projected to have substantial health benefits on a population level, according to Chen.</p>
<p>&#8220;Although this study was conducted among mostly overweight adults and many with hypertension, we believe that others will benefit by reducing the consumption of sugar-sweetened beverages,&#8221; she said.</p>
<p>&#8220;However, such evidence from humans is lacking, and we plan to conduct such research among non-hypertensive individuals.&#8221;</p>
<p>Researchers say further study &#8212; particularly randomized controlled trials to establish any cause and effect relationship &#8212; is warranted.</p>

His question was on the amount of what he was eating.  I took his question and ran with it though to a different topic.  Starvation mode dieting is never good, and similar is zero carb dieting.  I am pretty convinced that for most people going zero carb is an unnecessary step.  Sure there are some very metabolically resistant and / or people suffering with hyperinsulinemia.</p>
<p>For the most part, most people should be getting their veggies in and not fearing adding back a reasonable amount of carbs.  Trying to discover what the magic number per day can certainly be trying, but your health will appreciate it.  Thinking zero carbs is the answer also misses number of things:<br />
* phytonutrients without getting more chemicals<br />
* fiber-rich vegetables<br />
* carbs present in other non-veggie sources, like nuts, eggs, or cheese</p>
<p>Unless you are really hyper sensitive to carbs, enjoy the variety of nature and food selection.</p>

<p>I respond by saying that for most teenagers a lower carb approach is probably best, especially not if the choice is not driven by the child themselves.  I believe as parents we are called to teach our kids a healthy way of eating, and while acknowledging kids&#8217; environment and choices, we should make and help them choose to eat wisely, which by almost any standard should not include &#8220;whites&#8221; &#8211; refined starches and sugars.  There should be no requirement for breading or donuts to be healthy or happy.  The challenge with kids and teenagers is therefore school lunch programs and their unhealthy nature. </p>
<p>This is by no means a discouragement for teenagers, who may want to try Atkins Diet due to an internal desire to lose weight.  I just want them to be aware of what their decision means.</p>
